Thank you for your interest in our animal-assisted therapy program.  HHF will pay for 10 sessions provided the referral is made by a school counselor/psychologist; the only requirement of HHF is that the student attend weekly sessions.  Please request that your school counselor/psychologist complete the following form and return to us.

Once the student’s parent/legal guardian sets up the appointment, the therapist will contact us.  As you will note below, the student’s name is to remain anonymous.

To qualify for animal assisted therapy, a school counselor/psychologist must complete the online form. HHF will provide up to 10 sessions per student.
